As an Alzheimer’s Nurse Navigator (RN) , you’ll be the bridge between patients, families, and the clinical team—ensuring seamless coordination, education, and support from diagnosis through treatment and recovery.

 

💼 What You’ll Do:

• Serve as the key liaison for Alzheimer’s patients and their families
• Coordinate care across pre-op, acute, and post-care phases
• Educate, empower, and advocate for patients using evidence-based best practices
• Collaborate with a multidisciplinary team to enhance patient outcomes and care experience
• Lead performance improvement initiatives that make an impact

🎓 What You Bring:

• BSN or MSN (required)
• Active RN License (Compact or GA) + BLS Certification
• 3+ years of adult acute care experience (bariatric, spine, or case management experience a plus)
• Strong communication, leadership, and organizational skills
