Optimal pharmacological prescribed therapy, psycho- social aspects, barriers to discharge, and to determine appropriateness and alignment with the Opioid Stewardship criteria, monitoring and measuring outcomes for ongoing treatment. The Nurse Navigator will assist with the development and maintenance of a patient and prescriber needs, and will create and develop learning tools to educate, implement, evaluate, co-ordinate care and monitor outcomes of this program. The incumbent collaborates with multidisciplinary team members and liaises with health professionals and agencies within and external to the SHA to facilitate high quality care for Saskatchewan Stewardship patients.
